In an American Hospital Association , Carolyn Bogard, DNP, RN, discusses how El Camino Health joined the Age-Friendly Health System initiative because of the passion of a nurse, who implemented the initiative on her unit. She gathered data revealing high patient engagement scores for the large number of older adults on her unit. Already aware of data showing how the initiative reduces length of stay, readmissions, falls and other patient health outcomes, the leaders at the Mountain ÐÓ°ÉÔ­´´, Calif.-based health system agreed to implement the initiative throughout the system. Bogard, director of care coordination and palliative care, says the Age-Friendly Health System framework creates a platform to focus nurses’ passion for caring for older adults to produce tangible outcomes to streamline health care delivery.
